You will be asked to fill out our patient registration form and present your insurance cards for us to photocopy. If you wish to print out our forms and complete them at home, use the following links to obtain blank printable forms:

When you come for your appointment, please bring with you the following items:

Your current glasses or contact lenses.

A list of medications you are taking.

All of your insurance cards.

Sunglasses.

Someone to drive you home if you have trouble driving with your eyes dilated.

If you plan to discuss the treatment plan with a relative or friend, we encourage you to have them accompany you to your appointment.

If you have been referred by a physician or an optometrist, please inform us at the time of your visit and we will mail them a summary of our findings.
