Emergencies
An
ophthalmologist is available at all times to respond to your eye emergencies.
During evenings, weekends and holidays, you can reach the ophthalmologist on call
by calling the Emergency Service at 393-5522.
Most
eye problems can be handled in the office with more expertise and less expense
than at an emergency room or an urgent care center.

Directions
Our Holland office is conveniently located immediately north of Holland Community Hospital. If approaching
from the east on I-196, use the 16th Street exit. If approaching from the north
on U.S. 31, use 16th street.


Our Zeeland office is located at the interestion of 196 and State Street,
just west of Walter's Gardens.

Financial
Policy
If
you are a member of an HMO, a physician sponsored Medicaid plan or any other plan
which requires you to have a referral from your primary care physician, you must
arrange for your referral. Bring your referral at the time of your visit, or if
you prefer, you may have your primary care physician fax or mail it to us prior
to your visit.
We
participate with several insurance plans, and will be happy to bill them for any
medical service you receive when a medical condition exists. Routine eye
examinations are not covered by medical insurance unless your plan includes an
additional vision care rider.
If
we do not participate with your insurance plan, we ask that you pay for the service
at the time of your visit. We will provide you with a receipt that you can submit
to your insurance company for reimbursement.
Medicare
provides coverage for the medical portion of an eye examination, but you are responsible
for the refraction portion. The refraction is the part of the examination that
determines your prescription for glasses.
Our
insurance secretaries will be happy to assist you or answer any of your insurance
questions.
